Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative or rehabilitative health care needs.

Clients include frail elderly, younger clients with disabilities, and adults with chronic illnesses requiring supports to maintain/promote independence. The 7 Home Health team offices are located in geographically-based Community Health Centres (CHCs), across different Vancouver neighbourhoods. Groundspring serves Gastown, Chinatown, Strathcona and the Downtown Eastside neighbourhoods.
Programs and services at this location will operate under the name Groundspring, a reference to the groundwater and springs that once shaped the False Creek mudflats in this area. Guided by our commitments to anti-racism, equity, and culturally safe care, we see this new community health centre as more than a building. For many, it has the potential to be a doorway to belonging, stability, and healing, and for our teams, a place where meaningful, relationship-based care can continue to grow.
