In this role, you will provide patient-centered care designed to improve independence and quality of life directly in patients' homes. You will enjoy the autonomy of managing your own caseload, backed by an experienced interdisciplinary team that values your expertise and clinical input.
Responsibilities
~1 min read- →Patient Assessment: Evaluate clients' physical, emotional, and cognitive abilities using clinical observations and standardized tools.
- →Treatment & Education: Create personalized treatment plans to enhance functional abilities. Educate clients and their families on modifying daily activities, including the use of adaptive equipment and home modifications.
- →Documentation: Maintain accurate, timely records of evaluations, progress, and treatment plans in compliance with legal and ethical standards.
- →Advocacy: Advocate for clients' needs and support them in accessing community resources, services, and necessary accommodations.
Requirements
~1 min read- Education: Degree in Occupational Therapy from an accredited institution.
- Licensure: Valid Arizona state licensure as an Occupational Therapist (or license-eligible).
- Skills: Strong clinical assessment, diagnostic, and treatment planning skills. Excellent communication and interpersonal skills with a patient-centered approach.
- Physical Requirements: Ability to lift and assist patients as needed. Ability to stand for extended periods and perform physical tasks related to home therapy.
