As a Population Health Navigator I, you play a key role in supporting population health initiatives by coordinating care and resources in collaboration with Primary Care Providers and the healthcare team. Using a comprehensive, holistic, proactive and “Patients First” approach, you will help patients and their care partners navigate the Cleveland Clinic Health System, schedule appointments, connect to community resources and address health-related needs.
Requirements
~1 min read- Must be able to sit for prolonged periods of time.
- Possess good visual acuity through normal or corrected vision.
- Possess dexterity to operate a PC and other office equipment.
- Light driving to outside locations within the region.
- Follow standard precautions using personal protective equipment as required.
