Cumberland County Guidance Center is seeking a Full-Time Case Manager to join our PATH (Projects for Assistance in Transitioning from Homelessness) ProgramThis rewarding community-based position is ideal for professionals with experience in case management, behavioral health, mental health services, housing services, social work, community outreach, or human services who are passionate about connecting individuals with the resources they need to succeed.

As a PATH Case Manager, you'll serve as an advocate, care coordinator, and support system for individuals experiencing homelessness and serious mental illness, helping them access housing, healthcare, behavioral health treatment, public benefits, and other essential community resources.

Responsibilities

As a PATH Case Manager, you'll work directly with individuals experiencing homelessness, housing instability, serious mental illness, and co-occurring disorders to help them obtain stable housing and supportive services.

  • Conduct community outreach, engagement, and client advocacy activities
  • Develop and maintain Individual Recovery Plans (IRPs)
  • Provide ongoing case management and care coordination services
  • Assist individuals in obtaining permanent, supportive, transitional, and emergency housing resources
  • Connect clients with mental health, medical, substance use, vocational, employment, and educational services
  • Assist individuals with public benefits, entitlement programs, and financial assistance applications
  • Coordinate services with behavioral health providers, housing agencies, healthcare organizations, and community partners
  • Maintain timely, accurate, and compliant documentation
  • Support individuals in achieving long-term recovery, housing stability, and independence

Requirements

  • Experience working with individuals experiencing homelessness or housing instability
  • Experience supporting individuals living with serious mental illness (SMI) and co-occurring disorders
  • Knowledge of Housing First, Permanent Supportive Housing, and recovery-oriented service models
  • Experience with crisis intervention, care coordination, and service navigation
  • Experience utilizing Motivational Interviewing (MI) and trauma-informed care practices
  • Bilingual English/Spanish

  • Bachelor's Degree in Social Work, Human Services, Psychology, Sociology, Counseling, or a related field
  • Minimum two (2) years of experience in one or more of the following areas:
    • Case Management
    • Behavioral Health
    • Mental Health Services
    • Social Services
    • Homeless Services
    • Human Services
    • Community Outreach
    • Housing Services
  • Knowledge of housing resources, supportive housing programs, and public benefit systems
  • Strong communication, advocacy, engagement, and relationship-building skills
  • Ability to work independently within a community-based setting
  • Experience coordinating services across multiple systems of care
  • Valid driver's license with an acceptable driving record
