Responsibilities

→Develop and implement individualized treatment plans in coordination with other healthcare providers.
→Engage in crisis intervention and risk management when necessary.
→Offer psychoeducation to patients and their families as needed.
→Document clinical encounters promptly and maintain accurate patient records in compliance with organizational policies.
→Participate in case conferences, staff meetings, and professional development opportunities.

Requirements

Master’s degree in Counseling, Psychology, Social Work, or a related field.
Current licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), or equivalent in NYS. Recent Master’s level graduates (Limited Permit Holders) are considered.
Experience working in an integrated or primary care setting is preferred.
Strong knowledge of evidence-based therapeutic modalities (e.g., CBT, ACT, brief interventions).
Ability to work in a fast-paced, dynamic environment and manage a diverse caseload.
Excellent communication and interpersonal skills.
Commitment to culturally competent care and the ability to work with diverse populations.
