- Provide hands-on physical therapy treatments and exercises under the direction of a licensed Physical Therapist to improve residents' strength, mobility, balance, and functional independence.
- Assist residents with transfers, ambulation, positioning, and therapeutic activities while promoting safety, dignity, and confidence.
- Monitor residents' progress and communicate changes in functional status, tolerance, or condition to the Physical Therapist and interdisciplinary care team.
- Accurately document treatments, resident responses, and progress in accordance with care plans, facility policies, and regulatory requirements.
Requirements

- Strong communication and interpersonal skills with a compassionate, resident-centered approach.
- Ability to work collaboratively with physical therapists, nursing staff, residents, families, and other members of the interdisciplinary team.
- Previous experience in long-term care, skilled nursing, rehabilitation, or a similar healthcare setting preferred
- Must be able to communicate fluently in English - both verbally and written
