ComForCare Home Health Care - Brantford is looking for a compassionate and dedicated Registered Nurse to join their growing team in Brantford, ON. Responsibilities:

Conduct comprehensive patient assessments and develop individualized care plans
Administer medications, treatments, and nursing interventions in a home care setting
Monitor and document patient health status, progress, and changes in condition
Collaborate with physicians, healthcare teams, and family members to coordinate patient care
Provide patient and family education on health management, medications, and disease prevention
Perform wound care, IV therapy, catheter care, and other clinical nursing procedures
Maintain accurate and timely clinical documentation in compliance with regulatory standards

Requirements:

Current Registered Nurse (RN) registration with the College of Nurses of Ontario (CNO)
Previous home care, community health, or clinical nursing experience preferred
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal skills for patient and family interactions
Ability to work independently and manage a caseload with minimal supervision
Valid driver's license and reliable vehicle for home visits in the Brantford area
Current CPR/First Aid certification
