The Family Service Worker is responsible for providing entry level case management to clients and their families. The Family Service Worker works closely with families and classroom staff to promote family engagement and school readiness.
Responsibilities
~2 min read- →Welcome families into the program and encourage parents to become engaged in the program’s decision-making process through participation on committees, classroom activities and training opportunities.
- →Complete intake and needs assessments with each family.
- →Maintain the structure and policies of the program through orientation of participants by reviewing program rules, policies, and case management visits.
- →Family Service Workers working in CTI’s Early Learning Program will complete home visits with assigned families at least semi-annually.
- →Interview, evaluate and work closely with families to identify and build upon strengths and develop strategies to address barriers and concerns identified through the assessment process. Family Service Workers working in CTI’s Early Learning Program will formulate Individualized Family Partnership Agreements based on Program requirements.
- →Establish, evaluate and monitor goals for clients linked to measurable outcomes. Work collaboratively with interdisciplinary teams of collateral providers and partners to ensure coordination of services.
- →Advocate on behalf of clients.
- →Provide coverage at site/classrooms as deemed necessary.
- →Work closely with staff to integrate children with disabilities and/or behavioral issues into the CTI School Age or Early Learning Program, including providing specialized training to staff as needed.
- →Interact with all professional agencies that child is working with. Closely monitor all accommodations and outside professional recommendations to ensure appropriate execution of recommendations.
- →Family Service Workers working in CTI’s Early Learning Program will plan and facilitate Parent Committee meetings.
- →Recruit children and families to participate in the program, including children with diagnosed disabilities.
- →Monitor child attendance in accordance with attendance procedures and support families to maintain consistent attendance.
- →Gather and maintain child and family health and other documentation to ensure compliance with Local, State, Federal regulatory guidelines.
- →Assist families as children transition to and from schools to ensure community support.
- →Maintain accurate case records by documenting all services rendered including up to date case notes, telephone contact logs and referrals. Enter required data into the agency databases with reporting as required.
- →Provide emergency response management to address any crises or safety issues that may arise.

Requirements
~1 min read- Bachelor’s degree preferably in Human Services
- 3-5 years of experience working with low-income individuals or families
- Desire to work with low income, culturally diverse, and at-risk families and individuals.
- Excellent customer service, organizational, oral and written communication skills.
- Ability to manage multiple priorities and work in a fast-paced environment.
- Proficient computer skills required in basic computer software and data collection systems.
- Excellent crisis management skills.
- Ability to maintain accurate records and confidentiality of information.
- Excellent attention to detail.
- Proof of MMR and recent physical
- Ability to manage multiple priorities and work in a fast-paced environment.
- Successful CORI/DCF review prior to employment which includes fingerprinting
- Excellent crisis management skills.
- Valid Driver’s License
- Bilingual skills (English/Spanish, English/Khmer English/Portuguese)
- 3-5 years of experience working with low-income individuals or families within a school or center environment.
