The Medical Director/Supervising Physician provides clinical leadership and physician oversight for the interdisciplinary care team as part of the Care at Home Solutions program. The Medical Director partners closely with the Advanced Practice Provider (APP), Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), Pharmacist, Community Health Worker (CHW), and Care Navigators to ensure delivery of high-quality, evidence-based, patient-centered care for individuals with complex medical, behavioral, and social needs. The Medical Director provides physician supervision and consultation, supports clinical decision-making and care planning, oversees quality and patient safety initiatives, and collaborates with primary care providers to improve health outcomes, reduce avoidable utilization, and achieve performance goals within value-based care programs.  The Medical Director is responsible for managing four core pillars of the program: Annual Wellness Visits (AWVs), Transitions of Care (TOC), Complex Care Management (CCM), and Disease Management.

Responsibilities

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
  • Active and unrestricted California Physician License required.
  • Board Certification in Family Medicine, Internal Medicine, Geriatrics, or related primary care specialty required.
  • Minimum five (5) years of clinical practice experience, preferably in primary care, Medicare Advantage, or value-based care settings.
  • Experience managing complex, high-risk patient populations and chronic disease management programs preferred.
  • Experience with Medicare Advantage, risk adjustment, HCC documentation, and population health initiatives preferred.
  • Previous Medical Director, physician leadership, or provider supervision experience preferred.
  • Experience working within interdisciplinary care teams, including APPs, nurses, pharmacists, and community-based care personnel.
  • Experience with telehealth and virtual care delivery models preferred.
  • Proficiency with Electronic Medical Record (EMR) systems and healthcare technology platforms.
  • Population health management principles.
  • Medicare Advantage and commercial payer programs.
  • Risk adjustment and HCC documentation.
  • Quality improvement methodologies and clinical performance metrics.
  • Telehealth care delivery models.
  • Electronic Medical Record (EMR) systems and healthcare technology platforms.
  • Strong clinical leadership and ability to collaborate effectively within interdisciplinary care teams. 
  • Knowledge of value-based care, population health management, and Medicare Advantage programs. 
  • Excellent communication, relationship-building, and patient engagement skills. 
  • Strong analytical, problem-solving, and clinical decision-making abilities. 
  • Ability to balance quality outcomes, patient experience, and cost-effective care delivery. 
  • Commitment to patient-centered care, quality improvement, and regulatory compliance. 
  • Demonstrates professionalism, accountability, integrity, and sound clinical judgment. 
  • Adaptability and ability to thrive in a telehealth-enabled, team-based care environment.
