Solo With Supports Youth Coordinator

We are currently seeking a full time Youth Coordinator in the Solo with Supports (SWS) program.
Closing Date: May 4th, 2026
Salary: $51,410.00 – $57, 494.00 Annually
The Solo with Supports program is a community-based program that assesses and teaches life and living skills to teens requiring additional support as they transition to adulthood. The youth in the program may live in a mentor support home or in the Chimo 5 Transition Home.
Youth are assigned an SWS Youth Coordinator who manage a caseload, providing case management and hands on support through:
Helping program participants build the skills, knowledge, and confidence required to successfully transition into adulthood and thrive in their preferred community and living environment.
Co-creating, monitoring, and reporting on Safety Plans and Service Plans for each program participant on their caseload.- Working collaboratively with SWS team colleagues, family members, support network, service team professionals, and community partners to ensure a cohesive and unified plan of care and support
Qualifications:
- A diploma or university degree and a minimum of 2 years of experience in Child and Youth Care, Disability Services, or a related field such as Social Work, Psychology, or Community Rehabilitation.
Requirements:
- Eligible to work in Canada
- Clear, current (within 6 months) Police Information Check and Intervention Record Check.
- A valid AB Driver’s License, clean three-year abstract, three-year driving record with NO at fault claims, and access to a reliable vehicle
Physical requirements:
- Ability to lift up to 50lbs
Ability to physically interact with youth during recreation outings, etc.
Successful candidates will:
- Demonstrate care and compassion for young people.
- Possess a strong understanding of working with young people and adults who live with challenges in a variety of areas including developmental disabilities, mental health, attachment, addictions and barriers to accessing community resources.
- Vave knowledge of community resources to support youth
- Have strong teamwork and communication skills
- Be proficient with computer technology and writing reports/logs
