About the Role

~1 min read
  • Lead our care team and deliver supportive care to patients with cancer, including supportive oncology patient care, palliative care, second opinions, clinical trial advisory, and documentation integrity.
  • Care for high-acuity and clinically complex patients across our hybrid model, including home-based and virtual visits, and manage clinical escalations from the care team.
  • Participate in supportive oncology on-call coverage to support patients and the care team.

Team Leadership:

  • Provide day-to-day clinical supervision, mentorship, and case-based teaching for the market's Nurse Practitioners, RNs, and other clinical staff.
  • Participate in and lead interdisciplinary case conferences and huddles, ensuring complex patients have clear, well-communicated care plans.
  • Champion a culture of clinical excellence, continuous learning, and evidence-based practice within the care team.
  • Uphold Daymark's clinical policies, documentation standards, and quality measures in daily practice, and help the team consistently deliver on them.
  • Partner with the Market Medical Director and Daymark's national clinical leadership to refine care pathways, workflows, and programs, bringing frontline insight into how the care model performs for complex, seriously ill, and home-based patients.

WHAT WE ARE LOOKING FOR

  • You must be a board-certified physician (MD or DO) in medical, radiation, or surgical oncology, or a closely related specialty (such as hospice and palliative medicine, internal medicine, family medicine, or geriatrics) with an active, unrestricted California medical license (or willingness to obtain one), or an experienced Advanced Practice Providers (NP or PA) with demonstrated leadership track record in these areas. You have at least 5 years of experience practicing supportive oncology care, including direct patient care, and are comfortable being accountable for the clinical care of your teams.
  • You have significant experience managing clinically complex patients with cancer throughout the care continuum, including end-of-life care, and are comfortable leading serious illness and goals-of-care conversations. Experience delivering home-based or virtual care is strongly preferred
  • You are a natural mentor and clinical leader who elevates the practice of the clinicians around you, including NPs, RNs, and other care team members.
  • You are comfortable with technology, able to learn new systems and tools, and you thrive in a fast-paced environment where programs and processes are still being built and improved.
  • You're available 4+ days per week; a minimum of 3 days per week is required.
