Douglas Services LLC is seeking a compassionate, detail-oriented Licensed Practical Nurse to perform supervisory visits for individuals with intellectual and developmental disabilities receiving Community Living Supports and related services.


About the Role


As an LPN Supervisory Visit Nurse, you will conduct scheduled supervisory visits, observe client condition and service delivery, review staff performance, identify changes in condition, and communicate concerns to the RN Supervisor and agency leadership.


This position supports an agency licensed as a Private Home Care Provider under the Georgia Department of Community Health Healthcare Facility Regulation Division and serving individuals who may be deemed medically frail or medically compromised.


What You’ll Do


You will visit clients in their homes or community settings to ensure services are being delivered safely, appropriately, and in accordance with each individual’s service plan.


Key responsibilities include:


Conduct supervisory visits for assigned clients, including MF/MC individuals.

Monitor client condition, service needs, satisfaction, and overall safety.

Review whether direct care staff are following the current service plan.

Identify changes in condition, risks, service gaps, or documentation concerns.

Report clinical concerns to the RN Supervisor/RN Reviewer.

Support service-plan updates and follow-up as directed by the RN.

Observe and coach direct care staff when needed.

Ensure timely, accurate, and survey-ready documentation.

Assist with incident follow-up, complaint review, and quality assurance activities.

Promote dignity, independence, safety, and person-centered care.


Who We’re Looking For

We are looking for an LPN who is professional, organized, reliable, and comfortable working independently in community-based settings.


The ideal candidate understands that this role is more than checking boxes. It requires strong observation skills, good judgment, timely communication, and a heart for serving individuals with disabilities.


Requirements: Applicants must have:


-Current active Georgia Licensed Practical Nurse license.

-Ability to work under RN supervision and within Georgia LPN scope of practice.

-Experience in home care, private home care, IDD services, long-term care, or community-based nursing preferred.

-Current CPR/First Aid certification, or ability to obtain before assignment.

-Reliable transportation and valid driver’s license.

-Ability to travel to client homes and community locations.

-Strong written documentation and communication skills.

-Ability to recognize and report changes in condition.

-Ability to complete required background checks, onboarding, and annual training.

-Commitment to confidentiality, HIPAA, client rights, dignity, and safety.

Preferred Experience


Experience with any of the following is a plus:


Intellectual and developmental disability services.

NOW/COMP waiver services.

Private Home Care Provider services.

Medically frail or medically compromised individuals.

HRST-related risk indicators.

Supervisory home visits.

Incident follow-up or quality assurance.

Staff training or competency observation.


Flexible scheduling may be available based on agency needs and client assignments.


Position Type: Part-time, PRN, or Contract

Reports To: RN Supervisor / RN Reviewer and Agency Administrator
