We are seeing a dynamic and passionate Full-Time Mid-Level Medical Provider to lead, mentor and empower our Substance Use and Behavioral Health teams. This position will service the Eastern North Carolina areas.

Schedule: Primary work hours are Monday – Friday, 8:00 a.m. – 5:00 p.m., with additional flexibility as needed to support program operations.

Position Summary:

The Mid-Level Medical Provider plays a key role in supporting clinical excellence across Easterseals PORT Health teams. This role ensures that medical and nursing practices meet internal policies, regulatory standards, and community best practices. The Mid-Level Medical provider provides direct support, training, consultation, and clinical services, collaborating with interdisciplinary teams to promote integrated, high-quality care for individuals with mental health issues (including SPMI) and substance use diagnoses or co-morbidities.

  • Administer and document psychiatric medications in collaboration with the psychiatrist.
  • Monitor individuals for adverse reactions, side effects, and treatment effectiveness.
  • Provide structured, therapeutic, face-to-face interventions focused on psychosocial, rehabilitative, and functional domains.
  • Promote health education and medication adherence strategies in collaboration with clients and caregivers.
  • Coordinate care and communication with external medical providers as needed.
  • Educate staff on monitoring psychiatric symptoms and medication side effects.
  • Function as an active provider within the team, supporting client stability and community integration.
  • Attend team meetings daily/weekly to collaborate on positive outcomes for patients with the care team
  • Maintain accurate and timely documentation, including Medication Administration Records (MAR) and other clinical records.
  • Assist in preparing documentation for utilization reviews and audits.
  • Ensure compliance with national standards (e.g., those from the National Alliance for the Mentally Ill and SAMHSA).
  • Complete required training and certifications within prescribed timeframes.
  • Provide ongoing clinical support to care teams.
  • Assist with new staff training and promote consistent application of clinical standards.
  • Support integrated care and community collaboration initiatives.
  • Perform other duties as assigned and in accordance with Easterseals PORT Health policies.
 Qualifications:
  • Bachelor’s degree in Nursing (Master’s degree preferred)
    • Current unrestricted North Carolina license as a Nurse Practitioner or Physician’s Assistant.
  • Experience working with adults with SPMI and co-occurring substance use disorders preferred
  • Knowledge of adult psychopathology, psychiatric nursing, pharmacology, and community resources
  • Provide a variety of in person or virtual services in your home clinic and across the agency as needed or assigned.
