ACCOUNTABLE FOR: Freedom Healthcare LLC dba Community Wellness Outpatient Mental Health Center is seeking a Licensed Psychologist, PhD or PsyD, or a Psychology Associate who already has an approved clinical supervisor, to provide psychological testing, evaluation, test administration, scoring, interpretation, feedback sessions, and related outpatient behavioral health services on a fee-for-service basis.
CLASSIFICATION: Fee-for-Service (FFS), with the following classification options available:
- Fee-for-Service W-2
- Fee-for-Service 1099
WORK SCHEDULE: This role offers flexible scheduling based on provider availability, client need, testing referrals, office workflow, and agency requirements. The provider may work part-time, as needed, or on an agreed recurring schedule.
Testing appointments, feedback sessions, report completion timelines, and any therapy caseload expectations will be coordinated with Community Wellness Outpatient Mental Health Center and Enterprise Management Solutions LLC.
COMPENSATION STRUCTURE:
This is a fee-for-service role. Compensation is based on completed, documented, and approved services. Services may include, but are not limited to:
- Psychological testing and evaluation time.
- Test administration and scoring, first 30 minutes.
- Test administration and scoring, each additional 30 minutes.
- Psychological evaluation and feedback time.
- Report writing, when billable or approved under agency compensation policy.
- Therapy sessions, if the provider maintains a caseload.
- Other approved clinical services within scope.
Specific rates, payment timing, documentation requirements, and contractor versus employee classification will be determined during the onboarding or agreement process.

SUMMARY OF POSITION RESPONSIBILITIES
Freedom Healthcare LLC dba Community Wellness Outpatient Mental Health Center is seeking a Licensed Psychologist, PhD or PsyD, or a Psychology Associate who already has an approved clinical supervisor, to provide psychological testing, evaluation, test administration, scoring, interpretation, feedback sessions, and related outpatient behavioral health services on a fee-for-service basis.
The primary focus of this role is psychological testing and evaluation services, including the services shown below:
- Psychological testing, evaluation, and feedback by a physician or other qualified professional, including additional testing/evaluation time as clinically appropriate.
- Psychological test administration and scoring by a physician or other qualified professional for the first 30 minutes.
- Test administration and scoring by a physician or other qualified professional for each additional 30 minutes.
The provider may also maintain a small outpatient therapy caseload if needed, but the primary purpose of the role is to support psychological assessment, diagnostic clarification, evaluation, scoring, written reports, and feedback services for clients referred within the outpatient mental health center.
SCHEDULED DUTIES AND RESPONSIBILITIES
Psychological Testing and Evaluation
The provider will be responsible for completing psychological testing and assessment services, which may include:
- Conducting clinical interviews and diagnostic assessments.
- Reviewing referral questions and determining appropriate assessment batteries.
- Completing psychological testing for diagnostic clarification, treatment planning, level-of-care recommendations, and clinical documentation support.
- Administering psychological, behavioral, emotional, cognitive, developmental, and personality assessment tools within the provider’s scope of practice.
- Scoring, interpreting, and documenting testing results.
- Preparing comprehensive psychological evaluation reports.
- Providing feedback sessions to clients, families, guardians, referral sources, or treatment teams when clinically appropriate and authorized.
- Making clinically appropriate recommendations for therapy, medication evaluation, school supports, community services, higher level of care, or additional testing.
- Completing addenda, updates, or supplemental documentation when needed.
- Ensuring all testing, scoring, evaluation, interpretation, and feedback services are documented accurately and timely.
Test Administration and Scoring
The provider may perform or oversee:
- Initial psychological test administration and scoring for the first 30 minutes.
- Additional test administration and scoring in 30-minute increments when clinically necessary.
- Use of standardized testing instruments.
- Behavioral observations during testing.
- Validity checks and clinical interpretation of results.
- Coordination with other qualified professionals involved in the evaluation process.
- Clear documentation of time spent on test administration, scoring, interpretation, report writing, and feedback.
Clinical Documentation
The provider is responsible for maintaining complete, timely, and compliant documentation, including:
- Intake and assessment notes.
- Psychological evaluation reports.
- Testing protocols and scoring records.
- Diagnostic impressions.
- Treatment recommendations.
- Progress notes, when providing therapy.
- Feedback session notes.
- Medical necessity documentation.
- Time-based service documentation, where required.
- Any documentation required by Freedom Healthcare LLC, Community Wellness OMHC, Enterprise Management Solutions LLC, payors, licensing boards, or applicable regulations.
Outpatient Therapy Services, If Needed
Although the primary focus is psychological testing, the provider may also maintain a therapy caseload if clinically appropriate and mutually agreed upon. Therapy services may include:
- Individual therapy.
- Family therapy.
- Group therapy, if applicable.
- Crisis support within outpatient scope.
- Treatment planning.
- Diagnostic assessments.
- Progress notes and clinical updates.
- Coordination with psychiatrists, therapists, case managers, schools, primary care providers, and other care team members.
Collaboration and Care Coordination
The provider will work collaboratively with the outpatient mental health center team and may be expected to:
- Coordinate with therapists, psychiatric providers, clinical supervisors, administrative staff, and leadership.
- Participate in case consultation as needed.
- Communicate clinical findings in a clear and professional manner.
- Assist with determining appropriate referrals and service recommendations.
- Support clients and families in understanding evaluation results.
- Follow agency workflows for scheduling, documentation, billing support, and report submission.

The provider may perform services within their professional scope, license, training, and supervision arrangement, including but not limited to:
- Psychological testing.
- Psychological evaluation.

PHYSICAL DEMANDS:
- Prolonged periods of sitting, using a computer, reviewing clinical documentation, preparing psychological evaluation reports, and completing electronic health record entries.
- Frequent use of standard office and clinical equipment, including computers, telephones, testing materials, scoring tools, and secure documentation systems.
- Ability to communicate verbally and in writing with clients, families, clinical team members, administrative staff, and supervisors.
- May require occasional standing, walking, reaching, bending, or carrying testing materials, files, or office supplies.
- Must be able to maintain attention to detail during clinical interviews, psychological testing, scoring, report writing, and feedback sessions.
WORK CONDITIONS
- Outpatient mental health center setting with services provided through Community Wellness Outpatient Mental Health Center, operated by Freedom Healthcare LLC, with staffing placement coordinated through Enterprise Management Solutions LLC.
- Work may include in-person clinical services, psychological testing appointments, feedback sessions, documentation, report writing, care coordination, and administrative communication.
- The provider may work independently on a fee-for-service basis while coordinating with clinical leadership, supervisors, administrative staff, credentialing staff, billing staff, and other clinical team members.
- Work may involve interaction with clients experiencing mental health symptoms, emotional distress, behavioral concerns, trauma histories, family concerns, or complex clinical presentations.
- The role requires compliance with HIPAA, confidentiality standards, agency documentation policies, licensing board requirements, informed consent requirements, and professional ethical standards.
- Scheduling may vary based on provider availability, client need, testing referrals, office workflow, and agency requirements.

Qualification
For Licensed Psychologist Applicants
Applicants must have:
- Doctoral degree in psychology, PhD or PsyD.
- Active and unrestricted license as a psychologist in the applicable state of practice.
- Training and experience in psychological testing, evaluation, scoring, interpretation, and report writing.
- Ability to provide services independently within scope of license.
- Knowledge of outpatient mental health documentation standards.
- Ability to complete timely, clinically sound, and professional evaluation reports.
- Professional liability insurance, if working as an independent contractor.
- Ability to meet credentialing and onboarding requirements.
For Psychology Associate Applicants
Applicants must have:
- Active psychology associate status or eligibility to practice as a psychology associate in the applicable state.
- A clinical supervisor already in place and approved as required by the applicable licensing board.
- Documentation of supervision arrangement, if required.
- Training and experience in test administration, scoring, psychological assessment support, clinical interviewing, and report preparation within supervised scope.
- Ability to work within all supervision, licensing, and agency requirements.
- Ability to complete documentation and reports according to agency standards and supervisor requirements.
Supervision Requirements
Licensed psychologists must operate within the scope of their independent license.
Psychology associates must already have a clinical supervisor in place and must comply with all supervision requirements established by their licensing board, supervisor, and applicable agency policy. The agency may request documentation of the supervisory relationship before services begin.
Preferred Qualifications
Preferred candidates will have experience with:
- Outpatient mental health centers.
- Children, adolescents, adults, and families.
- Psychological testing for diagnostic clarification.
- ADHD, anxiety, depression, trauma, behavioral concerns, mood disorders, personality concerns, and developmental concerns.
- Writing clear, comprehensive, and clinically useful psychological evaluation reports.
- Working with Medicaid, commercial insurance, or behavioral health billing documentation requirements.
- Using electronic health records.
- Coordinating with therapists, psychiatrists, schools, primary care providers, and community agencies.
Education
- Doctoral degree in psychology, PhD or PsyD.
Licenses/Certifications
- Active and unrestricted license as a psychologist in the applicable state of practice.
- Active psychology associate status or eligibility to practice as a psychology associate in the applicable state.
