Outpatient Physical Therapist

Bring your passion for movement and meaningful care to Tallgrass Creek, part of Erickson Senior Living, is seeking a patient-centered Outpatient Physical Therapist to join our team and make a meaningful impact on the lives of older adults. In this role, you’ll deliver evaluations and personalized treatment across a full continuum of care—all within one vibrant, on-site campus. You’ll have the unique opportunity to expand your clinical expertise while building lasting relationships with residents. We set you up for success with dedicated onboarding and strong documentation support, so you can focus on doing your best work every day.

All of our clients live within our community, which means no additional travel!

• Caseload focused on primarily 1:1 direct patient care

• No outside travel - all residents live on our beautiful 63-acre campus

• Predictable weekday schedule.

• Excellent environment for new grads or PTs who enjoy continuity of care

• Long-term career growth within a respected national organization

• Focus on quality care, working alongside nursing, physicians, and rehab colleagues to help residents maintain mobility, independence, and quality of life

• Must possess a valid and current state Physical Therapy license. Degree and training must meet current Medicare and state requirements.

• Ability to document services accurately and on time

• Therapeutic experience in older adults across multiple diagnostic groups is preferred.
