• →Assist in developing and implementing individualized treatment plans for residents.
• →Support our CNAs and collaborate closely with our RNs and other team members to deliver exceptional care.
• →Oversee the Falls Program and Pressure Sore Program to ensure resident safety and well-being.
• →Monitor residents' health and promptly report any changed to the nursing team.
• →Perform additional duties assigned to support the best possible care for our residents.
• →Administer medications and treatments while offering reassurance and support.
• →Listen, comfort, and advocate for the residents - because here, every voice matters.
• →Build strong, trusting relationships with residents, their families, and your fellow team members.
• →Help create a safe, cheerful, and truly home-like environment for everyone.

• Active Licensed Practical Nurse (LPN) License in good standing.
• Experience in Skilled Nursing or Rehabilitation preferred.
• Strong teamwork and communication skills.
• Reliability - report to work on time when scheduled.
• A gentle, respectful approach when communicating with residents.
• Team spirit and a positive attitude.
