  Collaborate with medical providers, behavioral health staff, and front desk teams to manage patient referrals.

• →

  Schedule and track both internal and community referrals, including follow-up on appointments.

• →

  Review patient records from outside clinics and hospitals, including results, imaging, and other medical documentation.

• →

  Connect with patients in-clinic or via phone to coordinate appointments, transportation, or other needs.

• →

  Verify insurance eligibility and refer uninsured patients to community resources.

• →

  Manage voucher programs, including DI vouchers, cab vouchers, bus passes, and other patient support programs.

• →

  Assist with applications and placement for Skilled Nursing Facilities, Hospice Care, and Respite Care programs.

• →

  Acquire durable medical equipment, oxygen, or other necessary medical devices through insurance or charitable resources.

• →

  Maintain accurate tracking sheets and patient documentation to support continuity of care.

• Relevant experience working with underserved or vulnerable populations.

• Strong communication, interpersonal, and organizational skills.

• Ability to multitask, prioritize, and manage workflows effectively.

• Ability to take constructive feedback and work independently and collaboratively.

• Skilled in writing and documenting detailed patient information.

• Bilingual in Spanish.

• Experience with EPIC or other electronic medical records (EMR) systems.

• College education (associate degree or higher), or relevant experience in social services, psychology, case management, or direct patient care.
