We are seeking a compassionate and skilled Home Health Nurse (RN/LPN) in Naschitti, NM, to provide high-quality, one-on-one care in the patient’s home. The nurse will deliver care in accordance with the physician-established plan of care while ensuring the patient’s comfort, safety, and overall well-being.

~1 min readThis is a 1099 independent contractor opportunity, with services provided in the patient’s residence. The patient currently requires 8 hours of nursing care per week.

Responsibilities
~1 min read- →Provide compassionate, individualized nursing care in the patient’s home.
- →Follow and implement the physician-established plan of care.
- →Monitor the patient’s condition and communicate changes or concerns appropriately.
- →Advocate for the patient’s needs, comfort, safety, and quality of life.
- →Work collaboratively with CNAs, HHAs, Case Managers, physicians, and other members of the care team.
- →Build a trusting, professional relationship with the patient and family.
- →Maintain accurate and timely documentation of services provided.
Requirements
~1 min read- Current RN or LPN license in the state of New Mexico.
- Navajo language proficiency is required.
- Compassionate, dependable, and patient-focused approach to care.
- Ability to work independently while collaborating effectively with the care team.
- Strong communication and clinical skills.
- Must successfully pass a background check.
