Giving Home Health Care is seeking a highly organized healthcare professional to serve as the primary non-clinical care coordinator for an assigned patient population. This role manages patient cases, coordinates communication among patients, caregivers, physicians, the Department of Labor (DOL), and internal teams, and helps remove barriers to care. The ideal candidate brings experience in case management, social services, patient navigation, or healthcare care coordination rather than direct-care caregiving.
  • Manage a caseload of approximately 40-50 patients, depending on acuity and case complexity, and serve as the primary point of contact throughout the patient journey.
  • Coordinate services and communication among patients, caregivers, physicians, referral sources, the
DOL, and internal clinical and operational teams.
  • Monitor patient progress, complete follow-up and case review activities, and identify barriers affecting
services, caregiver support, or patient satisfaction.
  • Advocate for patients with the DOL by conducting phone calls and completing other advocacy-related
tasks to support patient needs and resolve barriers.
  • Complete patient onboarding, education, scheduled outreach, home visits, and physician-appointment
    support as needed.
  • Maintain timely documentation, protect patient confidentiality, and comply with company policies, HIPAA
    requirements, and applicable standards.
  • Escalate complex clinical, operational, or service concerns to the appropriate team or leader.
  • Minimum of two years of experience in healthcare case management, care coordination, patient
navigation, social services, discharge planning, healthcare operations, or a related field.
  • Strong communication, relationship-building, organization, critical-thinking, and problem-solving skills.
  • Ability to independently manage a patient caseload and coordinate across multiple stakeholders and
    priorities.
  • Proficiency with Microsoft Office and electronic documentation systems.
  • Valid driver's license, reliable transportation, and ability to travel locally to patient homes, physician
    offices, and company locations.
  • Bachelor's degree in Social Work, Healthcare Administration, Human Services, Public Health,
Psychology, Nursing, or a related field preferred but not required.
  • Three or more years of relevant case management, patient navigation, social work, or care coordination
    experience.
  • Experience with home health, workers' compensation, chronic disease management, population health,
    physician offices, or community resources.
  • CCM, ACM, Social Work licensure, or a comparable professional credential.
  • Company holidays, floating holiday, and birthday holiday
  • Vacation (PTO) and Sick Days
  • Competitive benefit plans
  • Company paid STD and LTD
  • Tuition reimbursement program
  • Monthly cell phone reimbursement
  • 401(k) with matching
  • Being a part of a growing company!
  • Working with incredible patients and their families!
