✓Provide compassionate, direct patient care under the supervision of a Registered Nurse following each patient's individualized plan of care.
✓Deliver treatments, monitor patient conditions, and report changes promptly to the RN Care Manager.
✓Assist with pain and symptom management to promote comfort and quality of life.
✓Educate patients and family caregivers about diagnoses, medications, treatments, and the plan of care.
✓Accurately document all patient care, assessments, and interventions in the electronic medical record.
✓Collaborate with physicians and the interdisciplinary team to coordinate exceptional hospice care.
✓Safely prepare and utilize medical equipment and supplies while following infection control and safety standards.
✓Participate in interdisciplinary team meetings, staff meetings, continuing education, and quality improvement initiatives.
✓Support Good Samaritan's mission by providing compassionate, respectful, and patient-centered care in every interaction.
✓Participate in the nursing holiday rotation.
✓Graduate of an accredited Practical Nursing program.
✓Current Virginia Licensed Practical Nurse (LPN) license.
✓Minimum of three (3) years of nursing experience; hospice, home health, community health, or medical-surgical experience preferred.
✓Experience with electronic medical records (EMR).
✓Current driver's license, reliable transportation, and acceptable driving record.
✓Current CPR certification (or ability to obtain per organizational requirements)
