Registered Nurse - RN Case Manager

• →Serve as a key member of the Interdisciplinary Team (IDT), providing holistic, patient- and family-centered hospice care
• →Complete comprehensive nursing assessments, admissions, and ongoing reassessments in accordance with hospice regulations and timelines
• →Develop, implement, and continuously evaluate individualized plans of care in collaboration with the IDT and attending physicians
• →Provide skilled nursing care, including symptom management, pain control, wound care, IV therapy, and physical assessments
• →Educate patients and families on disease progression, medications, pain and symptom management, and routine caregiving tasks
• →Provide emotional support and compassionate guidance to patients and families throughout the hospice journey
• →Fulfill on-call responsibilities as assigned and assist with admissions or coverage during staffing needs

• Currently licensed as a Registered Nurse in the Commonwealth of Virginia
• Minimum of two (2) years of nursing experience
• • Hospice or home health experience preferred
• Strong clinical judgment, critical thinking, and decision-making skills
• Excellent communication skills with the ability to educate, support, and collaborate effectively
• Proficiency with EMR documentation and computer-based systems
• Valid driver’s license, reliable transportation, and proof of automobile insurance
