Licensed Practical Nurse (LPN)

• →Provide nursing care and support to individuals in their homes and in the community, in a respectful, person-centered way
• →Administer medications and treatments according to each person's care plan, and track and report any medication errors
• →Train and coach direct support staff on medication administration, CPR, and recognizing signs of illness
• →Keep medical records complete, accurate, and current
• →Communicate with physicians and other providers to relay updates, request orders, and get clarification when needed
• →Step in for hands-on direct care when needed, including help with daily activities, home tasks, and medication administration

Required:

• Current Minnesota LPN license in good standing
• Valid driver's license and ability to pass a driving record check
• Ability to pass the Minnesota Department of Human Services Background Study

Preferred:

• Comfort with basic computer programs (Microsoft Office) and willingness to learn digital charting tools
• Experience in home and community-based services or a similar care setting
