As an RN Case Manager, you will provide skilled nursing care in the home while guiding each patient’s plan of care from start to finish.

• →Assess, plan, implement, and evaluate individualized patient care
• →Develop care plans in collaboration with physicians and the interdisciplinary team
• →Complete admissions and ongoing nursing assessments
• →Educate and support patients and families through changing needs
