Virginia Home Care Services is seeking a dependable, compassionate, and clinically competent Full-Time Registered Nurse (RN) Field Nurse to provide skilled nursing services to patients in the comfort of their homes throughout Northern Virginia.

We are looking for a nurse who is confident in clinical assessment, organized with documentation, and committed to delivering high-quality, patient-centered care.

Position Responsibilities

Perform skilled nursing visits in patients' homes in accordance with physician orders and individualized plans of care.
Complete Start of Care (SOC), resumption of care, recertification, routine skilled nursing, and discharge visits as assigned and qualified.
Conduct comprehensive assessments and complete required OASIS documentation accurately and timely.
Monitor patient conditions, identify changes in health status, and promptly communicate concerns to the appropriate clinical personnel and physicians.
Perform medication reconciliation and provide patient and caregiver education.
Manage chronic conditions, assess wounds, monitor treatment responses, and provide other skilled nursing interventions within the nurse's competency.
Coordinate care with physicians, therapists, caregivers, and the interdisciplinary team.
Maintain timely, complete, and compliant electronic medical record documentation.
Promote patient safety, continuity of care, and prevention of avoidable hospitalizations.
Follow Medicare requirements, Virginia regulations, and agency policies.

Qualifications

Current, unrestricted Registered Nurse license in Virginia.
Home health experience preferred; experienced nurses with strong clinical skills are encouraged to apply.
OASIS and Medicare home health documentation experience preferred.
Strong clinical assessment, critical-thinking, and time-management skills.
Excellent communication and organizational abilities.
Ability to work independently while collaborating with the clinical team.
Valid driver's license, reliable transportation, and willingness to travel throughout the assigned territory.
Ability to use an electronic medical record system and complete documentation within required timeframes.
