Overview Specialty: Family Medicine / Internal Medicine Location: Bolton Area, Connecticut Practice Setting: Office-based outpatient Primary Care Work Schedule: Full-time weekdays; four ten-hour days are offered. Support Staff / Resources: Dedicated Medical Assistant and clinical support; Behavioral Health, high-risk nurse care management, Social Work, and specialty referrals EMR: Epic with Dragon and Ambient AI; electronic patient communication tools New Grads/Practicing: New graduates welcome to apply. What You Can Expect Provide prevention, acute assessment, and longitudinal chronic disease management. Coordinate specialty referrals and community-based follow-up. The practice serves patients from birth through adulthood.
