Healthcare United is seeking a Nurse Practitioner for a full-time role focused on Geriatric Medicine in the Bolton Area, Connecticut. Deliver longitudinal Geriatric care in patients’ homes and assisted living settings with multidisciplinary support. Overview Specialty: Nurse Practitioner / Geriatric Medicine Location: Bolton Area, Connecticut Practice Setting: Patient homes and assisted living facilities Work Schedule: Full-time weekdays only, with scheduling flexibility. Support Staff / Resources: Multidisciplinary Geriatric care team EMR: Epic Compensation: Competitive compensation. What You Can Expect Assess and manage older adults in home and assisted living environments. Coordinate treatment, referrals, and follow-up with a multidisciplinary team. Use Epic to support continuity across care settings. Requirements Family, Adult, or Adult/Gerontology Nurse Practitioner training. Unrestricted Connecticut license or eligibility. At least two years of postgraduate NP experience required. The two years of experience must be in Geriatric Medicine or a related field. Current CV required.
