Responsibilities

→Deliver primary care services for pediatric, adult, and geriatric patients
→Perform wellness visits, preventive screenings, and routine checkups
→Diagnose and manage acute and chronic health conditions
→Create and adjust individualized treatment plans, including patient counseling
→Maintain accurate, timely documentation in the EMR system
→Coordinate care with specialists and facilitate smooth care transitions
→Participate in quality improvement and performance initiatives within the clinic

Requirements

Current Arizona NP or PA license
Graduate of an accredited Nurse Practitioner or Physician Assistant program
Active DEA registration or ability to obtain
Meets credentialing requirements for Medicare, Medicaid, and commercial insurance plans
Prior outpatient primary care experience preferred; motivated new graduates welcome to apply

AZ NP or PA License Required
US NP/PA Program Completed (Not in Puerto Rico)
