About the Role

~1 min read- →Provide direct, in-person care at assigned SNF and ALF facilities
- →Maintain an on-site presence daily
- →Participate in weekend rotation once or twice per month
- →Manage 15–16 patient visits per day, including new and follow-up appointments
- →Respond to nursing staff calls related to your assigned patients
- →Conduct assessments, adjust medications, and manage chronic conditions
- →Collaborate with facility staff and interdisciplinary teams
- →Document all encounters accurately using PointClickCare (PCC) EMR
Requirements
~1 min read- Active, unrestricted Arizona NP or PA license
- Graduate of an accredited U.S.-based NP or PA program
- Preferred: 1–2 years of post-acute, long-term, or primary care experience
- Strong communication, clinical reasoning, and organizational skills
- Reliable, professional, and comfortable working independently
- Current and unrestricted Arizona Nurse Practitioner or Physician Assistant license
- National certification (AANP, ANCC, or NCCPA)
