Active Nurse Practitioner or Physician Assistant license in Arizona (or eligibility in progress)
Graduate of an accredited U.S.-based NP or PA program (Puerto Rico programs not accepted)
Current or eligible DEA registration
BLS certification required; ACLS preferred
Minimum 1 year of primary care experience preferred

Valid Nurse Practitioner or Physician Assistant license in the State of Arizona (or in process)
Completion of an NP/PA Program in USA (Not Puerto Rico)

We are seeking a Nurse Practitioner or Physician Assistant to provide full-spectrum outpatient primary care to patients across all ages. This role emphasizes continuity of care, patient education, and improving health outcomes within our community.

Responsibilities

→Conduct comprehensive physical exams and review diagnostic results
→Diagnose and treat acute illnesses (e.g., infections, minor injuries)
→Manage chronic conditions including diabetes, hypertension, heart disease, and obesity
→Develop and implement individualized treatment plans
→Provide patient-centered care addressing physical, emotional, and social needs
→Collaborate with physicians, nurses, and specialists for coordinated care
→Educate patients and families on conditions, treatment options, and wellness strategies
→Coordinate referrals, follow-ups, and hospital admissions when necessary
→Maintain accurate, timely documentation in the EMR
→Lead wellness screenings and preventive health initiatives
→Maintain current clinical knowledge through continuing education
