Case Planner Youth & Families (PINS)

Nassau County, NY | Field-Office-Remote Role | 35-hour Workweek

Are you passionate about supporting at-risk youth and strengthening families? Hope for Youth is looking for a dedicated and dynamic Case Planner to join our Nassau County PINS Diversion Program!

Our Person in Need of Supervision (PINS) Diversion Program aims to prevent court involvement and out-of-home placements for youth under 18 by providing short-term, family-centered, in-home support throughout Nassau County. We focus on building problem-solving skills and linking families to long-term community resources.

As a Case Planner, you will support youth and families by improving family dynamics, encouraging behavioral change, and reducing the risk of placement outside the home. This role requires flexible hours, including evenings and weekends, and a field-based presence using your own vehicle.

• →Carry a caseload of 15–25 families
• →Conduct weekly in-home visits with youth and families
• →Deliver life coaching, parent skills training, and case management services
• →Attend school meetings, court appearances, and service planning sessions
• →Complete and update CANS-NY assessments
• →Maintain accurate, timely documentation and progress reports
• →Participate in team meetings, supervision, and ongoing staff development
• →Adhere to HFY and regulatory standards and practices

• BSW or Bachelor's in a related field with 2+ years of casework experience, or MSW
• Strong written, verbal, and tech skills
• Bilingual (Spanish/English) preferred
• Valid NYS Driver's License & access to reliable transportation with mileage reimbursement available
• Ability to work a flexible schedule to accommodate families
• Physically able to perform duties (e.g., drive own vehicle, climb stairs, prolonged periods of walking/sitting)
