About the Role

  • Conduct comprehensive psychosocial and clinical assessments of patients.
  • Develop, implement, and regularly review individualized treatment plans.
  • Assist patients in identifying strengths, coping strategies, resources, and recovery goals.
  • Participate in interdisciplinary case reviews and communicate clinical recommendations.
  • Provide individual, group, and family therapy as clinically appropriate.
  • Facilitate therapeutic and psychoeducational groups addressing mental health, coping skills, emotional regulation, substance use, relationships, and other relevant topics.
  • Utilize evidence-based and patient-centered therapeutic interventions to support recovery.
  • Provide crisis intervention and assist with safety planning when clinically indicated.
  • Assess and respond appropriately to changes in patient needs, risk factors, and clinical presentation.
  • Participate in interdisciplinary treatment team meetings and communicate clinical recommendations.
  • Collaborate with nursing, medical, case management, social services, and other members of the treatment team.
  • Coordinate aftercare services, including outpatient mental health treatment, residential programs, substance use services, and community resources.
  • Coordinate discharge planning and provide appropriate referrals and continuity-of-care recommendations.
  • Maintain accurate, timely, and legally compliant clinical documentation.
  • Ensure all assessments, treatment plans, and discharge plans meet hospital and regulatory standards.
  • Participate in audits and quality improvement initiatives.
