About the Role

  • Encourage patients to attend groups and actively participate in treatment programs.
  • Monitor and assist patients when they are not participating in treatment programs.
  • Assist patients with telephone use in accordance with facility policy.
  • Always accompany patients during smoke and outside breaks.
  • Assist nursing staff during medication pass times.
  • Report patient concerns to appropriate leadership and ensure appropriate follow-up.
  • Monitor patients according to ordered observation levels, including 1:1, Q5, Q15, and line-of-sight precautions.
  • Perform patient room searches and surveillance rounds in a timely manner.
  • Perform routine safety rounds and monitor compliance with hospital safety rules.
  • Maintain a safe and therapeutic environment for patients, staff, and visitors.
  • Activate the Emergency Management Plan when conditions require.
  • Report unsafe conditions to leadership.
  • Assist with patient intake, including required screening and body searches.
  • Perform patient discharge duties, including gathering and accounting for patient belongings.
  • Accurately complete discharge checklists and forms as directed.
  • Document patient belongings inventories in the electronic medical record.
  • Ensure shift notes are completed accurately and timely.
  • Document shift notes, safety rounds, patient observations, and other required activities in the EMR.
  • Communicate patient concerns and changes in condition to appropriate members of leadership and the treatment team.
  • Maintain HIPAA compliance and protect patient confidentiality.
  • Ensure compliance with patient care quality standards and applicable regulatory requirements.
  • Follow OSHA requirements and established workplace injury and employee illness procedures.
  • Perform appropriate hand hygiene throughout the shift.
  • Use personal protective equipment (PPE) appropriately.
  • Identify opportunities for performance improvement and actively participate in ongoing Performance Improvement (PI) activities.
  • Support initiatives designed to improve patient care, safety, and overall quality of services.
