Social Worker

The Social Worker provides emotional support and linkage to community resources for patients and families, based on an ongoing Social Worker Assessment of Need.  The Social Worker plans, implements, and evaluates care through systematic assessment of psychosocial needs and participates in community support groups and bereavement care.

The responsibilities of this position enumerated below, involve significant physical activities including standing, sitting, lifting (up to 50 pounds unassisted), bending, stooping, pushing, pulling, twisting, manual and finger dexterity and eye-hand coordination.  Requires the ability to operate office equipment, computers, and the office telephone system. May be exposed to infectious waste/diseases, to include but not limited to TB, AIDS, COVID, and Hepatitis B viruses.

DUTIES:

• As part of the Comprehensive Assessment, the Psychosocial Assessment is to be completed within five days of admission.  All other documentation is to be completed within 48 hours.
• Conducts regular Social Worker visits following the initial assessment according to the needs of the patient and/or the family.
• Unless all contact has been declined, the Social Worker will make at least monthly visits or phone calls.  With each contact, the Social Worker will document any needs and plans for follow-up. 

• Perform other duties as assigned

QUALIFICATIONS NEEDED:

• Master of Social Work Degree required
• Must be a Licensed Social Worker
• At least one year’s experience in healthcare setting
