*Advisor-Advance Care Planning

Assists with program development and system wide implementation of Advanced Care Planning (ACP). Provides ACP expertise to produce quality outcomes and promote evidence based practice in collaboration with the patient, family, providers, and other medical professionals. Initiates the opportunities for ACP conversations. Assesses the needs of individuals seeking advance care planning assistance, providing a person-centered approach to ACP based on the individual's health, readiness, fears, concerns, and beliefs. Aids in promoting ACP as a standard of care across the health system. Provides staff and community education programs to promote understanding of ACP.

• Bachelor's Degree is required. Master's Degree is preferred.
• 5-7 years experience in the health care field.
• Requires license or board certification within two years of hire date. 
• Requires a valid Indiana license as a Social Worker, Chaplain, or Registered Nurse with an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
