This is an excellent opportunity for an experienced RN to join a collaborative and patient-focused primary care team in Muncie, Indiana. Supporting a unique practice that combines Family Medicine Obstetrics (FMOB) and Internal Medicine under one roof, you will play a key role in providing care coordination, chronic disease management, patient education, medication management, referral coordination, care gap closure, and post-hospitalization follow-up for patients across the lifespan, including infants, obstetric patients, families, and adults. Working alongside 10 providers, 2 dedicated triage nurses, and 11 clinical support staff, you will help drive continuity of care and positive patient outcomes in a busy and rewarding ambulatory setting. This full-time day shift position offers a consistent schedule of 8:00 a.m. to 4:30 p.m., no travel requirements, and the opportunity to make a meaningful impact within a highly supportive team environment.

Qualifications:

• Requires an Associates of Nursing (ASN); Bachelors of Nursing (BSN) preferred.
  Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
• Requires that the RN has graduated from a nationally accredited nursing program. Exceptions: Candidates who have not graduated from a nationally accredited program may be considered if they possess more than two years of relevant clinical experience. Such exceptions are subject to review and approval by the Chief Nursing Officer (CNO).
• Requires Basic Life Support certification through the AHA.
• Other advanced Life Support certifications may be required per unit/department specialty according to patient care policies.
• May require ability to assess patients without face-to-face interaction.
• Requires awareness and understanding of legal implications of patient care activities and performance as a Registered Nurse.
