Position Title: Social Worker-MSW LCSW

Position Summary
The Social Worker - MSW/LCSW meets face-to-face with patients and families at Riley Hospital and serves as a resource and role model for students, interns, LBSWs, and LSWs. This role partners with the clinical team to develop treatment plans that address psychosocial issues, consults with other providers on mental health diagnoses, and collaborates across the care team to ensure patients and families receive safe, timely services and necessary resources and education. The Social Worker also leads on complex discharge planning, serves as the point person for abuse, neglect, and domestic violence cases, and provides specialized mental health services such as crisis intervention and de-escalation, including in dangerous or acute crisis situations.
Key Responsibilities
Meets face-to-face with patients and families.
Assists in the development of educational materials for team members, patients, and families.
Serves as a resource and role model for students, interns, LBSWs, and LSWs.
Participates with the clinical team on the development of treatment plans to resolve psychosocial issues and meet psychosocial needs, including consulting with other providers on mental health diagnoses.
Effectively communicates and collaborates with all members of the health care team to facilitate the episode of care so patients receive safe, timely services and all necessary resources and education.
Interprets and communicates complex patient/family needs and role models caring practices to other caregivers.
Assesses patients' and families' psychosocial risk factors, including prior functioning levels, support systems, reaction to illness, and ability to cope.
Counsels patients and families regarding the emotional, social, and financial consequences of illness and/or disability, and accesses and mobilizes family/community resources to meet identified needs.
Provides mental health assessments of patients upon physician referral.
Provides therapeutic intervention to facilitate complex discharge planning.
Serves as the point person in child abuse/neglect, adult/elderly abuse/neglect, institutional abuse, and domestic violence.
Serves as liaison between Indiana University Health and state and county child and adult protective teams.
Maintains principles and practices of crisis intervention and de-escalation techniques.
Provides intervention in guardianship (temporary/permanent), foster care, adoption, and mental health level II placements.
Provides specialized mental health services, including crisis intervention and addiction assessments, with recommendations for psychiatric referral or community resources.
Assists with dangerous and potentially dangerous situations and takes a leadership stance to provide clinical intervention.
Supports patients undergoing acute crisis, using de-escalation techniques to avoid episodes of physical violence.
Utilizes effective interviewing principles with alleged victims, perpetrators, family members, and significant others.
Provides intervention for end-of-life care, guardianship (temporary/permanent), foster care, and adoption.
Qualifications
Master's Degree in Social Work is required.
Permanent LCSW licensure is required.
Certification in Crisis Prevention Intervention or an alternative de-escalation training is required within 6 months of hire.
Knowledge of evidence-based psychosocial assessment, short-term therapeutic counseling, and crisis intervention for common mental health and substance use disorders is required.
Knowledge of the disease process and human behavior in relation to health-related social needs is required.
Complex discharge planning skills to provide safe discharge to the next level of care are required.
Knowledge of state and county adult and child protective legal mandates is required.
Knowledge of the principles of growth and development across the life span, including end-of-life care, is required.
Ability to identify decision makers and coordinate and execute goals-of-care conversations to provide excellent care and manage throughput is required.
Experience adapting services to geographically, culturally, and financially diverse populations is required.
Ability to develop and maintain strong working relationships with hospital administrators, medical teams, law enforcement, APS/DCS, schools, probation, and other community agencies as indicated is required.
Computer literacy and proficiency documenting in an electronic medical record required.
May be scheduled to work on-call and holiday rotation.
