(PT) Physical Therapist (Full-time) - The Wellness Center

• →Evaluate and assess patients to diagnose musculoskeletal conditions and movement dysfunctions 
• →Develop and implement individualized treatment plans tailored to each patient’s needs 
• →Administer therapeutic exercises and manual therapy techniques to improve mobility and function 
• →Educate patients and their families on injury prevention, fitness, and home exercise programs
• →Collaborate with physicians and other healthcare professionals to coordinate comprehensive care
• →Oversee the work of a physical therapy assistant
• →Document patient progress, treatment outcomes, and comply with state regulations and reporting requirements
• →Communicate with workers’ compensation carriers on the patient’s progress and ability to return to work 
• →Complete disability forms for the State of Indiana 
• →Other duties as assigned 

• Doctor of Physical Therapy (DPT) degree from an accredited program
• Physical therapist license from the Indiana Professional Licensing Agency

• Minimum of two (2) years of experience in an inpatient or outpatient physical therapy clinic
• Fulfill continuing education requirements to maintain licensure

• Problem solving skills; resourcefulness
• Experience with electronic medical records
• Attention to detail

• Excellent customer service skills
• Ability to take initiative and problem solve without direction
• Stamina and dexterity
• Compassion
