• →Coordinates entire arc of Discovery process for assigned participants.
• →Works with participants to identify internship and employment opportunities, selected through Discovery process.
• → Coordinates the transition to employment activities for assigned individuals.
• →Participates in recruiting participants for the project.
• →Maintains accurate records for customer, participant, and organizational needs.
• →Establishes and maintains an individual service file for all assigned participants. Implements an ongoing data‐collection system for each, as identified by the Program Manager. Takes notes on participant performance and problems. Records ISP / IEP data and ensures that ISP/ IEP objectives are accomplished.
• →Engages with employers participating; educates employers and staff; facilitates resolution of participant’s challenges.
• →Serves as liaison with community businesses, parents /guardians, and other external agencies.
• →Contributes to project’s strategic plan and outcomes.
• →Assists with participant behavior support. Provides coaching, counseling and feedback when necessary.
• →Assists in identifying creative solutions to encourage smooth transitions.
• →Facilitate internships and employment by providing all aspects of support.
• →Onboarding participants into internships
• →Drives personal vehicle to alternative work sites. Occasionally transports participants.
• → Other related duties.
