This position offers the opportunity to make a meaningful difference in the lives of older adults through personalized support and efficient clinical operations. It is ideal for someone who combines healthcare experience, strong communication skills, and a genuine passion for patient advocacy.
Working Schedule: Full-time, 40 hours per week, Monday to Friday, 9:00 AM–5:30 PM Eastern Time. Weekend availability may occasionally be required based on operational needs and member demand, with advance notice whenever possible. Optional overtime may also be available.
Care Coordination and Order Management: Coordinate medical services and clinical orders across healthcare providers, home health agencies, pharmacies, diagnostic facilities, and specialist networks to ensure timely and effective patient care.
Home Health and Medical Equipment Coordination: Work with home health agencies to establish Start of Care (SOC) dates, process skilled nursing orders, coordinate Durable Medical Equipment (DME) requests, and track authorizations through successful delivery and completion.
Laboratory and Diagnostic Imaging Management: Coordinate laboratory tests and diagnostic imaging requests, monitor electronic results and incoming documentation, and ensure information is routed promptly to clinicians for review to prevent delays and potential patient safety risks.
Referral Management: Manage inbound and outbound referral workflows, identify appropriate in-network specialists, coordinate required authorizations, and obtain relevant medical records to support continuity of care.
Prescription and Medication Support: Assist with prescription refill requests, route medication clarification inquiries to the appropriate providers, and escalate adverse events or clinical questions according to established protocols.
Clinical Team Support: Collaborate with healthcare providers to collect medical records, gather patient information, schedule and confirm appointments, and manage outgoing communications.
Preventive Care and Quality Improvement: Review patient panels and disease registries to identify outstanding preventive care needs and support the closure of STARS/HEDIS quality gaps.
Preventive Screening Coordination: Help coordinate A1c monitoring, blood pressure checks, colorectal cancer screenings, diabetic retinal eye exams, mammograms, and Kidney Health Evaluations (KED).
Patient Education: Explain the importance of preventive screenings and facilitate access to in-home testing kits, mobile diagnostic services, and clinic appointments to improve patient participation and healthcare quality outcomes.
Patient Advocacy and Communication: Serve as a reliable point of contact for patients through inbound calls, outbound outreach, digital messaging platforms, and operational task queues, providing empathetic and responsive assistance.
Healthcare Navigation: Translate complex medical instructions, healthcare benefits, and care plans into clear, accessible information for patients and their families, helping them navigate fragmented healthcare systems.
Patient Onboarding and Engagement: Review internal program referrals, initiate welcome communications, facilitate virtual patient onboarding, and distribute relevant educational resources and care kits.
Cross-Functional Collaboration: Work closely with physicians, advanced practice providers, registered nurses, pharmacies, family caregivers, and other healthcare professionals to coordinate care effectively.
Operational Flexibility: Support different clinical programs and operational workflows as needed, including primary care, acute care, and longitudinal care services, while adapting to changing organizational priorities.
Important scope clarification: This position does not involve independent clinical decision-making or activities requiring professional licensure. All delegated clinical tasks must be performed under the supervision and guidance of licensed healthcare professionals.
