This is a high-impact population health role connecting clinical expertise, quality improvement, analytics, and partner relationships. You will serve as a trusted advisor to partner organizations, helping them translate ACO quality data into practical strategies that improve patient outcomes. Using risk stratification and population health analytics, you will identify care gaps, uncover opportunities, and turn insights into measurable action. The role combines data-driven problem solving with meaningful collaboration across clinical and executive stakeholders. You will work closely with quality improvement teams and clinical leadership to influence workflows, training, and interventions. You will also help strengthen existing partnerships and identify opportunities to expand services and patient populations. This is an ideal opportunity for an experienced RN who is equally energized by healthcare data, quality improvement, technology, and relationship building.

  • Build and maintain trusted relationships with partner quality improvement teams and key clinical representatives, serving as a primary subject matter expert and problem-solving partner.

  • Use ACG and population health analytics platforms to stratify patient populations, identify care gaps, uncover risk patterns, and surface opportunities for improved outcomes.

  • Develop and maintain baseline reporting for ACO quality measures across partner populations, ensuring stakeholders have a clear and current view of performance and improvement opportunities.

  • Analyze quality and population health data to identify trends, risks, and opportunities, translating complex findings into actionable recommendations for clinical and business stakeholders.

  • Develop data-driven quality improvement strategies for targeted populations and collaborate with clinical leadership to implement workflow, training, and intervention changes.

  • Track the effectiveness of improvement initiatives and help connect quality interventions with measurable outcomes and performance improvements.

  • Monitor partner populations, emerging healthcare challenges, and regulatory changes to identify opportunities for new services, expanded patient panels, and stronger partner outcomes.

  • Support partner renewals and program expansion by clearly communicating the value and impact of population health and quality improvement initiatives.

  • Serve as a subject matter expert on ACO quality measures, population stratification, and population health improvement strategies for internal and external stakeholders.

  • Help establish repeatable processes for tracking partner needs, quality opportunities, regulatory shifts, and potential service expansion.

Requirements

  • Active Registered Nurse (RN) license is required.

  • 5+ years of clinical experience, including meaningful experience in population health, quality improvement, ACO operations, or a closely related healthcare environment.

  • Strong subject matter expertise in ACO quality measures and population health quality improvement strategies.

  • Hands-on experience with the Johns Hopkins ACG platform or a comparable risk-stratification or population health analytics platform.

  • Strong analytical instincts and the ability to investigate healthcare datasets, identify meaningful patterns, and translate findings into actionable opportunities.

  • Demonstrated ability to build trust and deliver results with external partners, including senior clinical stakeholders and CMOs.

  • Strong communication, presentation, relationship-building, and stakeholder management skills.

  • Confident, empathetic, outgoing, and highly collaborative approach to working with clinical and executive audiences.

  • Ability to communicate complex population health concepts clearly to both technical and non-technical stakeholders.

  • Comfortable working independently, managing multiple priorities, and adapting quickly when stakeholder needs or conversations change.

  • Strong interest in healthcare technology and willingness to adopt and champion new tools and platforms.

  • CPHQ or another population health certification is a plus.

  • Experience introducing or championing healthcare technology, leading change management, or engaging clinical and partner audiences around new platforms is a plus.

  • Experience expanding partner relationships, growing patient panels, or launching new healthcare service lines is a plus.
