• →Assess social, emotional, and environmental factors affecting patient health and well-being
• →Develop and implement individualized social service care plans
• →Provide counseling, emotional support, and crisis intervention for patients and families
• →Connect patients with community resources, support services, and benefit programs
• →Advocate for patients' rights and help navigate healthcare systems
• →Facilitate support groups and educational programs
• →Participate in interdisciplinary care conferences and collaborate with healthcare teams
• →Coordinate services with external agencies and community organizations
• →Maintain accurate, timely, and compliant documentation of patient interactions and services

✅ Licensed Medical Social Worker in the State of Florida

✅ Strong communication and interpersonal skills

✅ Ability to work independently while collaborating effectively with interdisciplinary teams

✅ Knowledge of medical terminology, healthcare practices, and regulatory requirements

✅ Experience working with diverse patient populations and providing culturally sensitive care
