As an Outreach Health Navigator, you’ll help make community healthcare more accessible by building trusting relationships with community members, Legacy patients, businesses, and partner organizations. Your compassionate guidance will help people connect with prevention services, enroll in community health programs and primary care, and stay engaged throughout their care journey.

You’ll work both in community settings and as an integrated member of a collaborative clinical care team. Through education, navigation, referrals, testing, and care coordination, you’ll support individuals with HIV prevention or treatment needs, medication adherence, and safer sex practices. Your work will help ensure care is culturally sensitive, responsive, and connected to each person’s goals and circumstances.

This is a meaningful opportunity to grow your skills while making a difference in the health of the Beaumont community. You’ll contribute to services focused on HIV/STI testing, Pre-Exposure Prophylaxis (PrEP), Post-Exposure Prophylaxis (nPEP), HIV Treatment as Prevention (TasP), medication adherence, and general safer sex practices.

  • Build lasting relationships with community members, patients, businesses, and partner organizations.
  • Help people access prevention services, primary care, community health programs, and the broader Legacy care continuum.
  • Coordinate outreach, engagement activities, health screenings, and point-of-care testing for HIV and other STIs.
  • Join an interdisciplinary clinical care team that values collaboration and whole-person support.
  • Strengthen community healthcare access through education, prevention, referrals, and care coordination.
  • Develop hands-on expertise in health education, navigation, motivational interviewing, and community engagement.
  • Participate in grant-supported work when applicable; this position may be partially or fully funded by grant(s).
