About the Role

As a Care Coordinator, you will lead the delivery and continuous improvement of our Aged Care services across Rockhampton and surrounding areas. In this role, you will manage client referrals, support individuals as they commence services, and regularly review and update care plans to meet clients changing needs. You will coordinate home visits to ensure clients receive high-quality, person-centred care, while fostering strong relationships with clients, families, and key stakeholders.

You will also contribute to the effective coordination of the program by reviewing, enhancing, and maintaining operational and administrative processes. With a strong focus on client experience, you will help ensure clients and their families feel supported and confident in the services they receive, while supporting the continued growth and success of our Aged Care programs.

This is a Permanent Full Time opportunity based in Rockhampton, with travel across the region to visit clients in their homes as required. Opportunity for a motivated professional to take ownership of the role and further develop their career in the aged care sector.

Responsibilities

  • Conduct client assessments and develop care plans with clients, representatives, and stakeholders
  • Update care plans regularly and conduct home visits
  • Monitor clinical risks and client needs, ensuring appropriate referral pathways are in place
  • Work collaboratively with internal teams, including Rostering and Practice Leads, to meet client needs
  • Develop strategies and/or broker services to address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities
  • Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community

Requirements

  • Demonstrated Aged Care clinical case management experience
  • Understanding of Aged Care funding and service delivery, specifically Support at Home
  • Confidence in utilising a variety of technology including mobile devices and computers
  • Proficiency with Microsoft Office, Outlook and database systems
  • Ability to effectively build relationships with diverse stakeholders
  • Strong communication skills
  • Ability to multi-task and work autonomously in a fast-paced environment
  • A current unrestricted Australian Drivers Licence, reliable vehicle and a willingness to travel
