About the Role

The residential program based in Fremantle provides vital community-based support to young people aged 17 to 22 who are experiencing homelessness, or are at risk of homelessness, and living with mental health challenges.

As a Mental Health Support Worker, you'll have the opportunity to make a meaningful difference in the lives of vulnerable young people. Working within a supportive and recovery-focused environment, you will provide psychosocial support that empowers clients to build confidence, strengthen independence, and work towards their goals with hope and determination.

This is a casual opportunity suited to flexible and reliable mental health support workers who can commit to a variety of shifts across a 24/7 roster, including:

Day shifts: 7:30am - 3:30pm
Afternoon shifts: 2:30pm - 10:30pm
Occasional active night shifts: 10:00pm - 8:00am

Responsibilities

→Develop and implement personalised support plans with consumers and key stakeholders.
→Collaborate with housing, mental health and community services to deliver coordinated care.
→Monitor consumer progress, review outcomes and provide timely reports.
→Build trusting relationships while understanding each consumer's goals and needs.
→Support access to relevant services, referrals and community resources.
→Maintain accurate case notes, records and documentation.
→Observe and respond to consumer needs, behaviours and wellbeing.
→Effectively manage workload and contribute to positive consumer outcomes.

Requirements

Certificate IV in Mental Health (or equivalent experience).
Experience supporting people with mental health challenges.
Experience working with people experiencing or at risk of homelessness.
Ability to work effectively with diverse communities, including Aboriginal and Torres Strait Islander peoples.
Strong engagement skills with people experiencing complex mental health needs.
Current First Aid Certificate.
Current Driver Licence.
