Our client in Salford is seeking an experienced Advanced Practitioner to join their Hospital Discharge Team, supporting adults with complex social care needs and ensuring safe, timely transitions from hospital settings. In this senior role, you will provide professional leadership, oversee complex casework, support practitioners through supervision and mentoring, and work collaboratively with partners to deliver high-quality, person-centred outcomes.

Responsibilities

In this role, you will,

  • →Undertake complex assessments and provide professional advice, guidance, and intervention in relation to individuals with complex social care needs.
  • →Provide professional leadership and expertise within the team, supporting high-quality, person-centred practice and decision-making.
  • →Manage and oversee complex cases, ensuring appropriate risk assessment, care planning, safeguarding, and review arrangements are in place.
  • →Provide supervision, mentoring, coaching, and support to social workers and other professionals, promoting reflective and evidence-based practice.
  • →Work collaboratively with individuals, families, carers, and multidisciplinary partners to achieve positive outcomes and promote independence.
  • →Provide specialist advice and guidance on complex safeguarding, legal, risk management, and social care matters.
  • →Support effective decision-making in relation to care and support arrangements, ensuring practice is consistent with relevant legislation, policies, procedures, and professional standards.
  • →Contribute to service development, quality assurance, audits, and the implementation of practice improvements within the service.
  • →Promote strengths-based, strengths-focused, and person-centred approaches, ensuring individuals are actively involved in decisions about their care and support.
  • →Maintain accurate and timely case records and provide professional reports and recommendations as required.
  • →Deputise for senior colleagues where appropriate and contribute to the effective management and development of the service.
  • →Keep up to date with changes in legislation, policy, research, and best practice, sharing learning and promoting continuous professional development across the team.

Requirements

To be successful in this role you must have:

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK driving licence
  • Post-qualified experience
  • Degree level or equivalent in Social Work
