Occupational Therapist - ASC Transformation

Our client in Nottingham City Council is looking for an experienced Occupational Therapist to join its Adult Social Care Transformation Team. This role offers the opportunity to undertake complex reviews and reassessments across a range of service areas, including older people, physical disabilities, learning disabilities, autism, mental health, and supported living.

You'll work with citizens, carers, and partner organisations to review care packages, assess changing needs, promote independence, and support strengths-based outcomes in line with the Care Act 2014. The team also plays a key role in service improvement and transformation initiatives across Adult Social Care.

What will your responsibilities be? 

• Complete complex reviews and reassessments across Adult Social Care
• Assess care and support needs using a strengths-based approach
• Manage cases involving moving and handling, safeguarding, and risk management
• Review high-cost care packages and identify opportunities for improved outcomes
• Recommend equipment, assistive technology, and adaptations where appropriate
• Work collaboratively with service users, families, carers, and professionals
• Contribute to service improvement and transformation projects

Qualifications & Experience: 

• HCPC Registered Occupational Therapist
• Adult Social Care experience
• Strong working knowledge of the Care Act 2014
• Experience with moving and handling, equipment, and complex assessments
• Excellent communication, analytical, and decision-making skills
