Social Worker - Community Health & Social Care

Liquid Personnel is recruiting Social Worker for its client's Community Health & Social Care Team at Devon.

You will manage a varied caseload of adults with complex needs, delivering high-quality, person-centred social work practice. Working closely with health colleagues, carers, families, and partner agencies, you will play a key role in safeguarding individuals and promoting independence within the community.

• →Undertake strengths-based assessments and develop personalised care and support plans
• →Manage a varied and complex caseload
• →Complete safeguarding enquiries and risk assessments
• →Apply legislation including the Care Act 2014 and Mental Capacity Act 2005
• →Work collaboratively with health professionals and partner agencies
• →Maintain accurate and timely case recordings
• →Make informed, defensible decisions that deliver positive outcomes

To be successful in this role you must have, 

• Social Work England registered Social Worker
• Relevant experience within Adult Social Care
• Strong assessment, safeguarding, and risk management skills
• Ability to work independently and as part of a multidisciplinary team
• Excellent communication and relationship-building skills
