Social Worker - Community Mental Health

Liquid Personnel is currently recruiting for an experienced Social Worker to join a Community Mental Health Team (CMHT) based in Dorset. This is an excellent opportunity to work within a supportive multidisciplinary team, supporting adults with complex mental health needs across the community while benefiting from flexible hybrid working arrangements.

As a CMHT Social Worker, you will undertake a varied caseload involving Care Act duties, safeguarding, mental health assessments, and partnership working with health professionals and external agencies. The role requires regular community and home visits across Dorset and offers the opportunity to make a genuine impact on the lives of vulnerable adults.

What will your responsibilities be?

In this role, you will:

• Complete Care Act assessments, support plans, and reviews.
• Undertake Section 117 assessments and reviews.
• Assess and support adults with complex mental health needs.
• Manage safeguarding enquiries, risk assessments, and complex casework.
• Work on cases involving MAPPA, MARAC, DoLS, and the Court of Protection.
• Collaborate with Community Psychiatric Nurses (CPNs), Psychiatrists, Psychologists, and partner agencies.
• Conduct community and home visits as part of your caseload responsibilities.
• Maintain accurate records and ensure statutory documentation is completed to a high standard.

Qualifications & Experience

To be successful in this role you must have:

• A recognised Social Work qualification (Degree/DipSW/CQSW).
• Current Social Work England registration.
• Previous experience within a Community Mental Health Team (CMHT) or Adult Social Care setting.
• Strong knowledge of the Care Act, Mental Health Act, and Mental Capacity Act.
• Experience managing complex caseloads and undertaking risk assessments.
• Excellent assessment, report writing, and communication skills.
• Ability and willingness to travel for community-based visits.
