Liquid Personnel is recruiting Social Worker for its client's Learning Disabilities Team at Middlesbrough.
Responsibilities
~1 min read- →Conduct strengths-based reviews of adults with learning disabilities living in Independent Supported Living (ISL) schemes, ensuring support arrangements promote independence, wellbeing, choice and positive outcomes.
- →Complete person-centred assessments and reviews, working closely with individuals, families, providers and the Accommodation Lead to evaluate support needs, identify risks and ensure placements remain appropriate.
- →Maintain accurate case records and review documentation, undertake home visits as required, and contribute to care planning and decision-making in line with Care Act duties, safeguarding procedures and local authority policies.

- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
