Adult Social Care Practitioner

Liquid Personnel is looking for a skilled and motivated Adult Social Care Practitioner to join its client’s multi‑disciplinary team. If you’re committed to empowering adults, promoting independence, and delivering high‑quality, person‑centred support, we’d love to hear from you.

In this role, you’ll work autonomously while contributing as a key member of the team. Your responsibilities will include:

• →Conducting Care Act 2014 Section 9 assessments and developing personalised support plans
• →Completing mental capacity assessments in line with the Mental Capacity Act 2005
• →Leading Section 42 safeguarding enquiries
• →Carrying out risk assessments and carers’ assessments
• →Working collaboratively on complex cases and ensuring high‑quality outcomes for service users

Our client is looking for someone with the professional confidence and expertise to make a real impact. You’ll need:

• Strong knowledge of the Care Act 2014 and Mental Capacity Act 2005
• The ability to act as a positive role model for best practice
• Experience offering formal supervision and providing guidance on hospital discharge planning, including funding and social care pathways
• The ability to think creatively when managing complex cases
• Strong organisational skills and the ability to manage your workload effectively
• Confident use of IT-based care management systems
• Excellent verbal and written communication skills
• A commitment to needs‑led services, quality standards, choice, and anti-discriminatory practice
• The ability to work within corporate policies and uphold Equal Opportunities principles
